Please read the Client Agreement below to ensure that you understand what is expected from both the client and the practitioner.
Client-Practitioner Agreement
Waiya Willow Wellness LLC
Welcome to my practice! Thank you for choosing Waiya Willow Wellness as part of your healing journey. I look forward to working with you and wish to take this opportunity to inform you about my practice, our work together, and our respective responsibilities.
My philosophy is that each client is responsible for their own healing and well-being and must be an active participant in any healing process. I help facilitate the client’s own healing using energetic balancing techniques. However, the client is ultimately responsible for their own health and well-being.
Practitioner Responsibilities
I will make every effort to be on time for client sessions. However, if I am running more that 5 minutes late, I will text client I will be late. If I need to cancel or reschedule an appointment, l will notify the client as far in advance as possible by text and/or email.
I practice with respect, honesty, integrity, sensitivity, and compassion and hold all information in strictest confidence.
I securely store all digital and written client data using standard security methods, software and reputable companies. No client information will be disclosed to outside parties unless required by law.
I am an energy-balancing practitioner—not a health provider nor an emotional therapist. If the client appears to require physical, emotional or mental help, I may recommend they seek a qualified professional.
I reserve the right to decline to work with anyone I feel that my abilities or the energy-balancing techniques I use are not appropriate for the client.
I am only available for sessions, phone, and emails during posted hours on my website. I do not provide any emergency sessions.
Client Responsibilities
You agree to take responsibility for your own healing and well-being. You agree to communicate honestly so that I may best serve you in our work together.
You understand and agree that you may experience tiredness and/or some temporary minor emotional/physical discomfort for several days after the provided service as your body clears and shifts energies in what is termed “processing”. You agree to rest and be gentle with yourself during this time in taking responsibility for your own health. If you experience anything more serious in the timeframe of using these energy rebalancing services, you agree to contact your healthcare provider or call emergency services.
You agree that all services provided are complementary methods to standard medical and mental healthcare. You understand and agree I only locate and release trapped energies; services provided do not diagnose, treat, or cure any ailment, nor are any of them a replacement for the diagnosis, treatment, or cure for any ailment. It is your responsibility to seek medical advice and services from qualified medical personnel for any physical, mental, emotional, and/or psychological issues that you experience in the time frame of using my services.
You understand and agree that each individual is unique and responds differently to these services. You agree that no guarantee is made to the number of sessions required to notice results, as this is individual to each client.
You agree that any mention of products, alternative therapies, and dietary information is strictly for educational purposes and is not a recommendation or an endorsement of the product, alternative therapy, or dietary information. You agree it is your responsibility to research and consult with your healthcare provider concerning the use of any of these products, alternative therapies, and dietary information.
You agree to hold Waiya Willow Wellness LLC and Teresa Damm from all liability for using these energy-balancing services or making decisions based on educational information discussed. You also agree that Waiya Willow Wellness LLC is an independent entity, and you agree that Discover Healing, Dr. Bradley Nelson, Discover Healing’s Program’s faculty/staff are not liable for Waiya Willow Wellness’s or its practitioner’s actions and/or statements.
Appointments & Payments
You agree to be on time for your scheduled sessions. If you are running late, you agree to notify me through email/text of how late you will be, and you agree to pay the full rate for shortened and/or missed sessions.
You must notify me at least 24 hours in advance if you need to reschedule or cancel an appointment. Failure to do so may result in termination of future service. You agree to pay the full rate for no-show sessions.
You agree to pay online for provided services within a day of the date stated on the invoice sent following each of your scheduled sessions. Failure to pay will result in ending our relationship and you will no longer be a future client. No refunds are provided.
You agree that it is your responsibility to notify the practitioner of any changes of personal contact information.
By checking the Client Agreement box during checkout, you have agreed to work with me and to abide by all of the above agreements. In addition, by checking the Client Agreement box you also confirm that you are over 18 years of age or the parent/legal guardian/owner of the client (child/pet/animal/property).
